The Social Service staff at Fieldston Lodge offers ongoing support and assistance to provide for the emotional well-being and concrete needs of all of our residents. Social workers provide guidance and counseling for residents and their families to ease the adjustment into a new way of living and to insure the highest quality of life for all those at Fieldston.

The social worker, as part of the greater interdisciplinary team, works with residents and their families to provide an individualized care plan for all residents. They serve as a bridge between residents and their families and coordinate the community resources they need. For residents receiving rehabilitation or nursing care in our sub-acute setting, the social worker facilitates connections with community resources or home care services to ensure that everything is in place for the patient and their family prior to being discharged back to the community. On a day-to-day basis, the department is driven to assist family members by arranging for desired everyday needs and addressing day-to-day concerns.
Whether providing information about available services or offering counseling and support, social services serve as a vital resource for both caregivers and residents at Fieldston Lodge.
